Below is the link to the 'Intake Packet' forms you will need to complete for your first visit. Please print AND complete all applicable forms and bring them with you to the first visit.

Clients cannot be seen until all intake paperwork is completed and signed. Please complete online through the therapy portal or bring it with you to the first session. Paperwork will take about 30 minutes to complete on-site.
Before a minor child can be seen, copies of the complete divorce documents are required. A child below the age of 18 is considered a minor. Minor children cannot be seen without these documents.
Insurance benefits must be checked in advance of your visit.
Texas Clients complete all intake paperwork online through the Therapy Notes Client Portal, print it off from the Forms link at the familyandyouthinstitute.com website, and email documents to info@familyandyouthinstitute.com before the visit, or bring a copy for our file with you to the first visit.

Credit Card
Completed Intake Paperwork
Biological Parent or Legal Guardian must accompany the child to the first visit.
Release of Information form for any other person who will be receiving information regarding the child.
Insurance Card
Drivers' License
A Complete copy of divorce documents or other legal documents granting rights to seek mental health services (Applies to minor children under age 18).
(Only necessary for when treating minor clients.) Forms also available through FYI Client Portal and can be completed online and signed through the Portal.
